For your convenience, there are several ways for you to request a copy of your 2016 Win Loss Statement:
Visit the Players Club and ask a representative to print you off a copy, which you will receive immediately.
From a self-serve kiosk at the Player’s Club, click on the “Win / Loss Statement” button and request that it be emailed to an email address of your choosing. It should arrive in your email promptly.
Request a copy by filling out the form below and it will be mailed to the street address currently associated with your Rock Star Rewards account. The name and Rock Star Rewards Account Number you enter on the form MUST match the information on your Rock Star Rewards account. Once confirmation of personal information is made, your statement will be mailed within 3-4 weeks.
